3/19/2023 EnchantaVaughn Approved Behavior: Upon arrival, client was waiting by the door to greet QMHP. Client appeared to be in an enjoyable mood, as evidenced by her calm and delicate facial expressions. QMHP-C engaged mother as well as client in a discussion about behaviors since the last session. Mother reports client needs to recognize her triggers before she lashes out on others in the home. Behavior: Client was attentive and followed along with the simple task. Client appeared to be retaining the information discussed by QMHP. Client cheered up and challenged herself to respond in a calm way; however, client needed some redirection to pay attention to the session as she was getting upset with her mother when discussing her anger issues. Intervention: QMHP initiated a rapport building conversation with client in efforts to learn more of her likes and dislikes as well as triggers to negative behaviors listed on her ISP. QMHP inquired of clientâs current mood. QMHP utilized active listening skills as client spoke of her current mood. QMHP will review key concepts of being a responsible child in the home. QMHP will educate client about being respectful, encouraging, mindful, and fair to peers and authority while making good decisions in the home. QMHP will teach client necessary coping skills to decrease impulses. QMHP will brainstorm strategies with client and encourage self-monitoring. QMHP listened to client explain why she reacts in the manners that she does. Intervention: QMHP educated client about what she can do to make her daily activities more fun and interesting, such as reading her favorite book, drawing in a journal, stretching and closing her eyes and practice the mindful behaviors learned in previous sessions. QMHP modeled proper positive behavioral expectations for the client to administer. QMHP assisted client in exploring other coping strategies to utilize in the home. Response: Client responds well to praise and support from QMHP. Client is acceptive of redirection from her mom. Client has improved with staying calm and utilizing relaxation skills in the home. Client responds well to feedback. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client said she was going to practice coping strategies and relaxation techniques in all aspects of her daily living routine in hopes that she will be a better individual. Response: Client is working on walking away and telling an adult before responding to anger provoking situations. Client understands that problem solving starts with thinking before acting. Client displayed an understanding of ADHD and its symptoms and causes. Client was observed and reported eliminating personal conflicts at home. Client utilized conflict management skills. Client exhibited techniques for appropriate self-expression and active listening. Client communicated positive self-talk and affirmations. Client was able to recognize personal strengths and abilities to perform better at home. Client stated some days she is extremely bored in the house, so she has challenges with certain activities and needs help staying focused.
